29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about 7/17/18, during residential siding activities located on Avon-Belden Road in North Ridgeville, Ohio, the employer failed to ensure that employees were provided, and utilized fall protection. The roof slope was about 4/12, at a height of about 11'.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 7/17/18, during residential siding installation located on Avon-Belden Road in North Ridgeville, Ohio, the employer failed to ensure that the Louisville extension ladder extended 3 feet above the landing surface of the roof. Employees were utilizing the ladder to gain access to the roof.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  On or about 7/17/18, during residential siding activities located on Avon-Belden Road in North Ridgeville, Ohio, the employer failed to ensure that the Louisville extension ladder was free from defects. The ladder had several dented rungs.
