29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    On or about July 17, 2018 at Precision Pump & Valve II, Inc, the employer failed to ensure that forklift operators were competent to operate the forklift. The employee used the forklift in order to load freight onto a truck.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv)(a): Flexible cords were attached to building surfaces.       On or about July 17, 2018 at Precision Pump & Valve II, Inc., the employer allowed employees to utilize an extension cord as a power source for a 1000 lb. chain hoist. The chain hoist was used to move valves.
