On or about July 17, 2018, at the residential job site, Lot 1336, in the Jerome Village-Willowbrush community, the employer did not ensure that the 28 foot Louisville aluminum extension ladder used by an employee to access the lower roof over the garage extended at least 3 feet above the upper landing surface to prevent the employee from falling approximately 12 feet, 1.5 inches.
