29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.  29 CFR 1910.305(d)(2): Panel boards were not dead front.  a) Facility - The 277/480V panel situated at the end of the aisle leading to the loading dock area was not dead front. A cover section was removed and the live terminals were exposed.  b) Facility - The 120/277V panel situated on the wall facing the loading docks was not dead front. On opening was cut in the cover, and live terminals were exposed.   c)  A recently installed new panel situated on the wall facing the loading docks was not dead front. The cover was not installed and all live parts were exposed.   d) Facility - The 480V 800A panel labeled Breaker #5 (also labeled Breaker #6) was not dead front. Sections of the cover were missing and live parts were exposed.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
