29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  The employer did not provide to the Agency, certification and documentation demonstrating that abatement was complete for OSHA Inspection number 1321567, citation 01, item 001b, which was due on 11/05/2018.     Violations occurred on or about 07/19/2017. inspection # 1321567: and the citations were received by employer on 10/12/18.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about July 19th, 2018, at site located at 8264 NW 48th Ter. Canarias at Downtown. Doral FL 33166, two employees were observed performing roofing activities on a roof at approximately 22 feet from the ground and were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 19th, 2018, at site located at 8264 NW 48th Ter. Canarias at Downtown. Doral FL 33166, the employer did not have two employees working from a high at approximately 22 ft. high, trained by a person qualified in the subject matter to recognize the hazards associated from falling and not using fall protection.
