29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s):  On or about July 19, 2018, in the shop area, employees were allowed to fabricate door frames with a Baileigh Tube Bender, which had its safety bypassed. The tube bender's foot pedal was held down by a weighted metal object, which caused the machine to run continuously, exposing employees to caught-in hazards.
