29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:     On or about July 18, 2018 during residential siding installation activities, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about July 18, 2018 employees were exposed to a fall hazards of approximately 19.10 feet.  The employees were installing siding materials on a residential home and employees were working on the east side of the home from a ladder jack scaffold without using any form of fall protection.
