29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about July 17, 2018, employees were exposed to a fall of approximately 10 feet while installing roofing materials on a residential home located at 1625 North State, Girard, Ohio 44420.  The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 17, 2018, employees were exposed to a fall of approximately 10 feet while installing roofing materials on a residential home located at 1625 North State, Girard, Ohio 44420.  The employees were working on all areas of the roof and the employer did not ensure that each employee was trained on how to recognize the hazards of falling or how to minimize them.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:    On or about July 17, 2018, an employee was involved in an accident that resulted in a concussion, multiple fractures, and an in-patient hospitalization. The employer failed to report the accident.
