On or about July 20, 2018, at a worksite located at Village Mall Drive, Ontario, OH, 44906, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.
