OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.    On or about July 18, 2018 at a salt water disposal facility in Kingfisher, Oklahoma, employees were exposed to severe burns during an operation to break up sludge in a storage tank.

29 CFR 1910.307(g)(2)(i): Locations were not classified depending on the properties of the flammable vapors, liquids, or gases that may be present and the likelihood that a flammable or combustible concentration or quantity is present.            This violation occurred on or about July 18, 2018, at the Devon Safari Saltwater Disposal Well site, the area below the unloading pumps, the trench, is in an area that had not been documented properly as  Class 1, Division 1.
