29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about July 19, 2018, at jobsite located on 10516 NW 81st Terrace, Summit, Building 10, Doral FL 33178, employees performing rebar and form installation were exposed to a 10 foot 5 inch fall hazard while working from the edge of a wall.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      On or about July 19, 2018, at jobsite located on 10516 NW 81st Terrace, Summit, Building 10, Doral FL 33178, employees performing rebar and form installation were exposed to fall hazards while using a ladder that was not extended 3 feet above the upper landing surface.
