(a) Worksite - 224 Plumb Beach Road, Saunderstown RI, 02874:  On or about 7/23/2018, the employer did not ensure that the safety latch on a hoisting hook used as part of a New Way mobile refuse collection system was in place and functioning properly prior to the truck going into service, resulting in the container slipping off the hook and striking an employee.
