One employee was installing sheets of plywood on a residential roof using a pneumatic nail gun without the use of eye protection.  This exposed the employees to struck-by injuries to the eyes.

One employee was securing panels of plywood in place on a residential roof without the use of fall protection . The employee did not have a harness and anchors were not installed.  This condition exposed the employee to falls from height.
