OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  (a) on or about 6/21/2018 a forklift operator was exposed to crushing hazards in that he was not using the seatbelt on the PIT  (b) on or about 6/21/2018 a second forklift operator was exposed to crushing hazards in that he was not using the seatbelt on the PIT, when he replaced the PIT operator specified in (a) who was removed by the employer because he had no PIT training in the last 3 years.
