29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment:    a)  Guardian Lima, LLC - Lima, Ohio:  On or about July 24, 2018, the employer did not ensure that written inspection and testing procedures were developed and implemented to ensure the mechanical integrity of pressure vessels, including but not limited to the Beer Column (T-400), Sieve Bottle 2 (T-433), and 200-Proof Rectifier (V-418).        b)  Guardian Lima, LLC - Lima, Ohio:  On or about July 24, 2018, the employer did not ensure that written inspection and testing procedures were developed and implemented to ensure the mechanical integrity of process piping, including but not limited to the following piping circuits: 3-P8300-19-C1 (KO to flare), 6-P4600-15-C1 (Sieve 1-3 to ET-4101 and ET-4102), and 6-P4400-09-C1 (EP-4402 to V-415).      c)  Guardian Lima, LLC - Lima, Ohio:  On or about July 24, 2018, the employer did not ensure that written inspection and testing procedures were developed and implemented to ensure the mechanical integrity of safety systems, including but not limited to pressure relief devices and process interlock devices.      The lack of written, implemented inspection and testing procedures can expose employees to fire/explosion hazards due to loss of containment of flammable liquids in the distillation process.

Lack of training in spill response and clean-up can result in chemical exposures and/or fire/explosion hazards.
