29 CFR 1926.106(a): Employee(s) working over or near water where the danger of drowning existed, were not provided with U.S. Coast Guard approved life jacket(s) or buoyant work vest(s)  (a) On or about July 24th, 2018 and at times prior thereto, the employer did not provide employees with U.S Coast Guard approved life jackets while working on the floating home at Duck's Moorage.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.   (a) On or about July 24th, 2018 and at times prior thereto, the employer did not ensure that equipment connected by cord and plug were visibly inspected each day for external defects.  This exposed employees to electrical shock while using the Black & Decker spade handle drill Model 1321.
