29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On July 24, 2018, located on the northeast side of an exterior roof structure, employees were installing sheathing on a low pitch commercial roof as there were no frequent and regular inspections being performed by a competent person to ensure the job-site, materials and equipment such as residential fall protection systems and/or devices were being maintained and utilized in a safe condition. Potential fall height measured to be greater than 11'-8" with a roof pitch measured to be 3 in 12.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On July 24, 2018, located on the northeast side of a commercial roof structure, employees were exposed to a fall hazard while sheathing a commercial roof while not utilizing any fall protection system. The potential fall height was measured to be 12'-8 with a walking/working surface pitch of 3 in 12.
