29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) New residential construction located on Lot #56, Cut bench located front side: On or about 07/19/2018, and at times prior, the employer exposed employees to severe laceration and/or amputation hazards, in that the Makita 7-1/4" Circular Saw did not have a functional lower guard as the saw was being used with a shim while cutting cornice boards.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. (a) New residential construction located on Lot #56, Roof Dormer: On or about 07/19/2018, and at times prior, the employer exposed employees to fall hazards of approximately 22 feet, 3 inches from the lower level, in that a fall protection system was not provided to employees while installing cornice boards on the pump jack scaffold system.
