29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  (a) Site: On or about July 24, 2018, the employer exposed employees to trip and fall hazards in that, pieces of cut lumber and OSB wood was littering the ground throughout the area while employees were framing a residential home.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Site: On or about July 24, 2018 and times prior, the employer exposed employees to fall hazards in that the aluminum extension ladder extended approximately 5 inches above the upper landing surface and exposed employees to falls of approximately 12.5ft to the ground. It was not secured at the top to prevent it from being deflected under a load while employees were gaining access to it to install roof trusses.
