29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Site: On or about July 24, 2018 and times prior, the employer exposed employees to fall hazards in that the aluminum extension ladder extended approximately 5 inches above the upper landing surface and exposed employees to falls of approximately 12.5ft to the ground. It was not secured at the top to prevent it from being deflected under a load while employees were gaining access to it to install roof trusses.
