29 CFR 1910.176(b): Storage of material created a hazard:  (a): Back of Warehouse - The employer did not ensure that stacked materials were stored properly to prevent collapse. Improperly stored materials exposed employees to struck-by and caught under hazards due to falling boxes from shelves, on or about August 23, 2018.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.176(c): Storage areas were not kept free from accumulation of materials that constituted hazards from tripping, fire, explosion or pest harborage:  (a): Throughout Entire Warehouse - The employer did not ensure that walkways and aisles were kept clean and free of debris. Employees are exposed to tripping hazards due to accumulated materials and improperly stored materials in common walkways, on or about July 23, 2018.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
