29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Worksite at 440 NW 23rd St. Oklahoma City, OK 73103. On or about July 24, 2018,  the employer did not provide and require the use of personal fall arrest systems, guard rail systems or safety net systems when walking/working on a surface with a unprotected side or edge which was 6 feet (1.8m) or more above a lower level, exposing the employee to a fall hazard.
