29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a. On or about July 26, 2018, on the eastside of the structure, an employee performing framing operations was using a pneumatic nail gun without safety glasses, exposing himself to being hit by nails or flying debris.   b. On or about July 26, 2018, on the eastside of a structure, an employee performing framing operations was using a Dewalt circular saw without safety glasses, exposing himself to being hit by flying debris.

29 CFR 1926.451(c)(2)(iii): Unstable objects were used as working platforms:  a. On or about July 26, 2018, on a piece of plywood that was sitting on the forks of a Genie GTH-844 forklift truck, an employee was elevated 30-foot, exposing himself to a fall hazard.
