(a) On the roof of the restaurant renovation project at 38 Commercial Road, Leominster, MA 01453, on 7/18/2018 and at time earlier employees were engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On the roof of the restaurant renovation project at 38 Commercial Road, Leominster, MA 01453, on 7/19/2018 and at times earlier employees were exposed to fall hazards up to 17 feet and the employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a) On the roof of the restaurant renovation project at 38 Commercial Road, Leominster, MA 01453, on 7/19/2018 and at times earlier employees were exposed to fall hazards up to 17 feet and the employer had not verified compliance with paragraph (a) of this section by preparing a written training certification record which included the name or other identity of the employee trained, the date of the training, and the signature of the person who conducted the training.
