29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:    (a)  Loveridge Builders LLC at 1735 Eisenhower Drive, Louisville, CO: On 7/25/18 and 7/26/18, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  Employees were re-roofing a two story home.  All employees were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  This condition exposed the employees to approximate 10.5 foot and 20 foot fall hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a)  Loveridge Builders LLC, at 1735 Eisenhower Drive, Louisville, CO: On 7/26/18, the employer did not ensure that portable ladders used for access to an upper landing surface had the ladder side rails extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  Employees were re-roofing a two story home.  The employees utilized an aluminum ladder to access the second story roof from the first story roof.  The ladder side rails extended approximately 1.5 feet above the upper landing surface.  This condition exposed the employees to an approximate 20 foot fall hazard.      (b)  Loveridge Builders LLC, at 1735 Eisenhower Drive, Louisville, CO: On 7/26/18, the employer did not ensure that portable ladders used for access to an upper landing surface had the ladder side rails extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  Employees were re-roofing a two story home.  The employees utilized a ladder to access the first story roof from the ground.  The ladder side rails extended approximately 1.8 feet above the upper landing surface.  This condition exposed the employees to an approximate 11.7 foot fall hazard.

Employees were re-roofing a two story home.  The employees utilized an aluminum ladder to access the second story roof from the first story roof.  The ladder side rails extended approximately 1.5 feet above the upper landing surface.  The ladder side rails straddled the first story roof ridge.  The ladder was not secured to prevent displacement.  This condition exposed the employees to an approximate 20 foot fall hazard.
