29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing hazards):     On 07/26/2014, the general contractor did not perform an adequate inspection of the commercial job site located at 3801 McClay, St. Peters, MO.  The general contractor or controlling employer had various fall hazards at their job site.  The following hazardous conditions existed:       a)  Five subcontractor employees were not protected by any means of fall protection while working on the roof of the structure.       b)   The access ladder to the roof for the subcontractors employees was not extended at least 3 feet above the upper landing (roof).
