29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Exterior, On the ground in the driveway:  Employees was exposed to head injuries while working below siding activities. The employees were not protected by hard hats or protective helmets, on or about June 27th, 2018.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   A) Exterior, On scaffold: Employees were exposed to fall hazards of approximately 23 feet while working on a ladder jack scaffold that was not protected by a complete guardrail system (including end rails)or any other means of fall protection, on or about June 27th, 2018.
