Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) At the jobsite - On or about July 26, 2018, employees working on the roof of a commercial building while performing roofing work were not provided with fall protection, exposing employees to fall hazards of up to 8 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Front of the building, on or about July 26, 2018, employees used a Davidson extension ladder that was extended 2.5 feet above the upper landing surface to access the roof of a commercial building during roofing work, resulting in fall hazards of up to 8 feet onto concrete floor.
