At the worksite - On or about July 26, 2018, employees worked from the roof while installing roof shingles without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 19.3 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed; or if the ladder's length did not allow the extension, the ladder was not secured at its top to a rigid support that will not deflect and provided with a grasping device such as a grabrail to assist employees in mounting and dismounting the ladder. a). At the worksite - On or about July 26, 2018, a Werner aluminum extension ladder (model: D1328-2) did not have its side rails extended at least three (3) feet above the roof's landing surface, thus exposing employees to a fall hazard of approximately 19.3 feet.
