At the worksite - On or about July 26, 2018, employees operated pneumatic nail guns without the use of eye protection, thus exposing employees to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  a). At the worksite - On or about July 26, 2018, employees worked from the roof while installing roof shingles without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 19.3 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed; or if the ladder's length did not allow the extension, the ladder was not secured at its top to a rigid support that will not deflect and provided with a grasping device such as a grabrail to assist employees in mounting and dismounting the ladder.  a). At the worksite - On or about July 26, 2018, a Werner aluminum extension ladder (model: D1328-2) did not have its side rails extended at least three (3) feet above the roof's landing surface, thus exposing employees to a fall hazard of approximately 19.3 feet.
