29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.    a). Front of the grocery store - On or about July 26, 2018, the employer did not instruct employees on how to recognize and control hazards associated with working in an area where vehicular traffic was present, thus exposing employees to struck-by hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    a). Front of the grocery store - On or about July 26, 2018, personal fall protection equipment was not worn by an employee working from the platform of a JLG Articulating Boom Lift (model: E400AJPN, serial #: 0300222227), thus exposing the employee to a fall hazard of approximately 25 feet.
