29 CFR 1910.67(c)(2)(i):  Lift controls were not tested each day prior to use to determine that such controls are in safe working condition.    On 7/24/2018, Weitkamp Tree Service, LLC did not ensure that employees operating the Altec LB546A/650A, Serial # 47-21154 aerial boom lift tested lift controls each day before use.  The lift controls had been damaged by falling trees and safety mechanism rigged with a tree branch.

On 7/24/2018, Weitkamp Tree Service, LLC did not ensure that each person was competent to operate an aerial boom lift safely, as demonstrated by the successful completion of training.  Two employees were operating Altec LB546A/650A, Serial # 47-21154 without required training.

1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift.     On 7/24/2018 Employees of Weitkamp Tree Service, LLC were working at a residence located at 650 2nd Street, Troy, MO.  An employee was working from an Altec LB546A/650A aerial lift truck at approximately 20-30 feet above the ground.  The employee was not wearing a personal fall arrest or travel restraint system exposing themselves to a fall hazard.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    On 7/24/2018, employees from Weitkamp Tree Service, LLC were exposed to caught-in hazards while operating a Woodsman 12x wood chipper.  The wood chipper had a missing door and a rope used to adjust the throttle/ idle exposing employees to rotating parts of the motor.
