29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about July 26, 2018, at the above addressed jobsite, employees were performing work on a dormer of a residential home and were not utilizing any means of fall protection. Employees were exposed to a fall hazard of approximately 10.5 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed: On or about July 26, 2018, at the above addressed jobsite, employees were performing work on a dormer of a residential home and misusing a straight ladder as a crawling board, at an unsafe angle, to access the roof and dormer. Employees were exposed to fall hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
