29 CFR 1926.453(b)(2)(v):   A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.  On 7/23/2018, an employee of Kaiser Construction, LLC was working on a building located at 1507 Highway Z in Wentzville, MO.  An employee was washing the side of a building while working from a Genie Aerial Lift, Model S-45.  The employee was not wearing body belt with a lanyard or personal fall arrest system that was attached to the boom or basket while working from the aerial lift.
