29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards:  At 3410 Wisconsin Avenue, St. Louis, MO, specifically at the northbound and southbound lanes, the employer did not post legible signs at the advanced warning area for the three day old work zone to ensure compliance with the Manual Uniform Traffic Code and Devices (MUTCD), 1988 Edition, Revision 3, or Part IV of the MUTCD Millennium Edition, which were incorporated by reference.  The KATO V-Four HD35 Backhoe, m/n 30V4, was positioned in the middle of the street.  This violation was observed on 07/27/18.

29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:  At 3410 Wisconsin Avenue, St. Louis, MO, there was an open trench measuring approximately 8'W X 12'L running east to west.  An unsupported, cast iron, natural gas pipe ran north to south parallel to the curb.  This pipe was not protected or supported as necessary to safeguard the employees.  This hazard was observed on 07/27/18.
