29 CFR 1910.176(b): Secure storage. Storage of material created a hazard. Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse: On and before 07/24/2018, at Eagle Nonwovens Inc., located at 10301 Lake Bluff Drive, St Louis, the employer exposed employees to struck-by hazards when it failed to stack bales in such a way that they were secure against sliding or collapse. The most recent instance of this occurred on 07/24/2018 when the employer stacked four 700 pound bales on top of one another against a wall at line #4. The bales were not stacked, blocked, interlocked and limited in height so that they were secure and stable. As a result, the bale on the top stack of four slid from the top, striking and killing an employee.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). On or about 06/25/2018, at the jobsite at 10301 Lake Bluff Drive, St Louis, MO, a Big Slitter operator was exposed to struck-by hazards when operating a powered industrial trucks without completing the formal instruction, practical training, and evaluation as required by the OSHA standard.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: On or about 07/24/2018, at the jobsite at 10301 Lake Bluff Drive, St Louis, MO, an employee on line #4 was exposed to struck-by and caught- in between hazards while operating a powered industrial truck. The operator received inadequate training in that he was not afforded enough time to perform practical driving exercises nor was there an evaluation of the employee operating the powered industrial truck. In addition, the employee had no prior forklift experience.
