29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design:     a) Jobsite: On July 24, 2018 and at times prior thereto an employee installing windows while standing on a sheet of oriented strand board (OSB) as his scaffold platform and elevated by a GEHL rough terrain forklift ; and the oriented strand board (OSB) was not secured to the forklift to prevent potential displacement.  The scaffold platform was not designed and constructed by a qualified person in that the platform was a sheet of oriented strand board and not secured to the forklift. The employee was exposed to an approximate 12 foot fall to the ground below.    Note) Certification of corrective action is required for this item.

The scaffold platform was not designed and constructed by a qualified person in that the platform was not secured to the forklift. The employee was exposed to an approximate 12 foot fall to the ground below. The employees did not recognize or acknowledge the hazard and take action that would control or minimize the hazard.
