29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:   On or about July 9, 2018 at 3106 Lanier Dr. NE Atlanta, GA 30319:  a) The employer did not ensure the area around the scaffold was free of debris including but not limited to, bricks, buckets, trash, scaffold parts, and empty cement bags exposing employees to tripping hazards.  b) The employer did not ensure that the porta potty was serviced and available for employees to use. The porta potty at the site was not serviced and filled with sewage.
