29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation.  On or about 07-25-2018 and times prior thereto, American States Utility Service, Inc. located at U.S. Army Base-Fort Lee, 16th Street and B Avenue Fort Lee, VA 23801:  Employee(s) were exposed to a trench collapse while working in accumulated water in the bottom of the trench during an active rain event.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety.  On or about 07-25-2018 and times prior thereto, American States Utility Service, Inc.  located at U.S. Army Base-Fort Lee, 16th Street and B Avenue Fort Lee, VA 23801:  Employee(s), while working in an unprotected trench were exposed to a collapse and not removed during wet and rainy conditions that would contribute to a collapse without trench protection.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
