OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  Location: 442 Avenue P, Newark, NJ.  a) The employer permitted the operation of a Kalmar Ottawa T2 (Unit No. 343178) spotter truck without a working audible back-up alarm or an observer as a ground guide while driving in reverse. The rear view from the driver cab was obstructed by the attached rear trailer unit. Employees were exposed to crushed-by and struck-by hazards, on or about 7/28/18.
