29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:    (a) On the northwest side of unit 3A:  On or about 8/2/2018, the pump jack scaffold was set up and used while only securing the scaffold to the top of the structure and not at the bottom.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    (a) On the northwest side of unit 3A:  On or about 8/2 2018, the employer did not train the employee who was working from the pump jack scaffold in that, the pump jack scaffold was not properly assembled and was being used in a manner that posed a fall hazard.
