29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Phillips Tube Group (d.b.a. Shelby Welded Tube) located at 5578 State Route 61 North in Shelby, Ohio; On or about July 13, 2018 the employer did not ensure employees were protected from the shear point created between the Southworth Lift Table and the table of Mill 8. Employees were exposed to caught in and amputation hazards while re-energizing the Southworth Lift Table.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Phillips Tube Group (d.b.a. Shelby Welded Tube) located at 5578 State Route 61 North in Shelby, Ohio; On or about July 13, 2018 the employer did not report to the local OSHA office of an employee hospitalization within twenty-four hours. An employee was hospitalized with multiple pelvis fractures on July 13, 2018 after the employee was pinched between a Southworth lift table and the bottom table edge of Mill 8. The Toledo area office was notified of the hospitalization on July 31, 2018.
