29 CFR 1910.1025(j)(1)(i): A medical surveillance program was not instituted for all employee(s) who were, or could be exposed to lead above the action level for more that thirty days per year:    (a)  Radiator Repair Area:  On August 20, 2018, an employee conducting radiator repairs was exposed to lead fume at an 8 hour time weighted average of 48.34 micrograms per cubic meter of air.  The employer has not instituted a medical surveillance programs for employees exposed above the action level of 30.0 micrograms per cubic meter of air.

29 CFR 1910.1025(l)(1)(ii): The employer did not train each employee who is subject to exposure to lead at or above the action level, or for whom the possibility of skin or eye irritation exists, in accordance with the requirements of 29 CFR 1910.1025:  (a)  Radiator Repair Area:  On August 20, 2018, an employee conducting radiator repairs was exposed to lead fume at an 8 hour time weighted average of 48.34 micrograms per cubic meter of air.  The employer has not trained the employee at least annually on the contents of 1910.1025(l)(1)(v)(A) to 1910.1025(l)(1)(v)(G).

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:    (a) Radiator Repair Area:  On August 2, 2018 and times prior, the employer did not ensure that all surfaces were maintained as free as practicable of lead.  Wipe samples indicated that 1161 micrograms and 128.5 micrograms of lead was present on the surfaces of the testing tub and tool box respectively.    (b) Office/Break Area:  On August 2, 2018 and times prior, the employer did not ensure that all surfaces were maintained as free as practicable of lead.  Wipe samples indicated that 2.388 micrograms and 1.706 micrograms of lead was present on the surfaces of the microwave buttons and soda machine buttons respectively.       West Virginia Spring and Radiator Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.1025(h)(1)), which was contained in OSHA inspection number 951497, citation number 1, item number 4b and was affirmed as a final order on 04/18/2014, with respect to a workplace located at 598 Viscose Road, Nirtro, WV  25143.
