Location: North River House, Port Liberte, Jersey City, NJ.  a.) Employees were exposed to amputations and other hazardous conditions when utilizing a power hand tool that had an abrasive wheel attached while not being equipped with the hand tool's protective guards; on or about 8/2/18.

An employee bypassed the guardrail system and was reaching over the unprotected edge as he was passing material/tools to employees working on the lower level.
