29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about August 2, 2018, at the north roof edge of a residential structure located at 3900 SE Lee Street in Stuart, Florida, an employee was exposed to a struck-by eye injury when operating a pneumatic nail gun while not wearing eye protection equipment.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about August 2, 2018, on the roof of a residential structure located at 3900 SE Lee Street in Stuart, Florida, employees were exposed to a fall hazard of approximately 9 feet - 6 inches when performing carpentry work on the roof of a residential structure while not being protected from falling to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about August 2, 2018, at the east roof edge of a residential structure located at 3900 SE Lee Street in Stuart, Florida, employees were exposed to a fall hazard of approximately 9 feet - 6 inches while gaining access to and from a roof when using an extension ladder that was not extended at least 3 feet past the roof edge and that was not secured to prevent accidental displacement.
