29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: a) On or about 08/02/18, at 2-story house located at 215 Ludington St., Buffalo, NY: An employee doing clean-up work as part of the roof replacement job on the flat portion of the roof at the front of the house was exposed to a fall of approx. 20 ft. to the ground because no fall protection system was being used. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 08/02/18, at 2-story house located at 215 Ludington St., Buffalo, NY: The siderails of the extension ladder used to access the flat portion of the roof at the front of the house only extended approx. 22 inches above the roof. NO ABATEMENT CERTIFICATION REQUIRED
