29 CFR 1910.147(c)(2)(ii): If an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section shall utilize lockout, unless the employer can demonstrate that the utilization of a tagout system will provide full employee protection as set forth in paragraph (c)(3) of this section.  On or about August 1, 2018, the employer failed to ensure that an employee who was servicing Line #11 for a faulty electrical contact on the extruder machine, utilized lock out tag out to protect the servicing employee from hazards such as but not limited to arc flash and electrical shock.  Employees are exposed to both pneumatic and electrical energy sources.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):  On or about August 1, 2018, the employer failed to ensure that an employee who was working on the electrical components of Line #11 had applied lock out tag out prior to performing the work.  The electrical energy was not fully isolated prior to performing the work and an employee sustained 2nd and 3rd degree burns to his hands due to an arc flash fire.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  On or about August 1, 2018, the employer failed to ensure that employees who work with or around electricity are provided with the appropriate personal protective equipment to protect them from hazards such as but not limited to electrical shock and burns.  An employee who was working on the electrical components of Line #11 had applied lock out tag out prior to performing the work, however, the electrical energy was not fully isolated prior to performing the work.  The employee also did not have on any FR rated (fire resistant) personal protective equipment.  An arc flash fire occurred and the employee sustained 2nd and 3rd degree burns to his hands.
