29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, or personal fall arrest systems:       The employer is failing to ensure employees are protected from fall hazards when conducting installation of shingles as part of the re-roofing operation of the home.  This was most recently documented at the job site located at 5202 Woodlane Dr., Papillion, Nebraska on or about July 31st, 2018 when the employer had employees working on the roof of this residence without any form of protective system.  Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  The workers were exposed to falls of over 22 feet to the ground below.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:        The employer is failing to provide employees with adequate and effective fall protection training prior to allowing them to work on a residential style roof. This was most recently documented at 5202 Woodlane Dr., Papillion, Nebraska on or about July 31, 2018 when the employer had employees working on the roof of this residence without any form of protective system and had not provided any training to them.  Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  The workers were exposed to falls of over 22 feet to dirt and concrete below.
