29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On Aug. 2, 2018, there were no frequent and regular inspections being performed by a competent person in that:    (a) employees were performing roofing work on a steep pitch commercial roof at fall heights greater than 15 feet (roof eave) and 30 feet (roof peak) above the ground below without a fall protection system     (b) two employees, while wearing ANSI-compliant harnesses without lanyards, were using the tool loops on the front of their work belts to tie off to a lifeline while working on a steep pitch commercial roof     (c) one employee, who was wearing an ANSI-compliant harness without a lanyard, was using a lifeline to rappel down the steep pitch roof to the next lower level of an adjacent building     (d) employees were using pneumatic nail guns to perform roofing work without donning eye protection

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    On Aug. 2, 2018, located on the northwest end of a commercial structure, employees were performing roofing work on a 12 in 12 pitch roof without a fall protection system. Potential fall heights were determined to be greater than 15 feet (roof eave) and 30 feet (roof peak) above the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   The employer did not provide an effective training program for employees that were performing roofing work on a steep pitch commercial roof in that employees, while wearing ANSI-compliant harnesses, were tying off to the loops of work belts or not tied off to any anchor point while not utilizing a fall protection system and exposed to fall heights greater than 15 feet (roof eave) and 30 feet (roof peak) above the ground below.
