29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. The employer is failing to ensure that all employees are protected from fall hazards when using fork-lifts with scaffold platforms that are not securely attached to the fork, preventing it from moving horizontally while the platform is occupied. This was most recently observed on July 31, 2018 at the jobsite located at the corner of 114th Street and Mercury Street Papillion, NE 68133. An employee was installing fascia on a two-story residential structure and utilizing a mobile scaffold with a platform inappropriately attached to the arm of the forklift. The employee was exposed to falls of over 18 feet to the ground level below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. The employer is failing to ensure that all employees are protected from fall hazards while working from elevated platform. This was most recently observed on July 31, 2018 at the jobsite located at the corner of 114th Street and Mercury Street Papillion, NE 68133. An employee was working on installing fascia on a new two-story residential structure. To accomplish this work activity, the employee was using a mobile scaffold with no fall protection system. The employee was exposed to falls of Approximately 18 feet to the ground level below.

This was most recently observed on July 31, 2018 at the jobsite located at the corner of 114th Street and Mercury Street Papillion, NE 68133 when the employer had employees walking on the top plate of the house being built on site.

The employer is failing to provide employees with adequate and effective ladder safety training prior to allowing them to work on a residential house. The top step of of the ladder on site was being used as a step to access the top plate of a wall. This was most recently observed on July 31, 2018 at the jobsite located at the corner of 114th Street and Mercury Street Papillion, NE 68133. The workers were exposed to falls of over 24 feet to the ground below. Had the employees had this training the above listed instances would not have been present on the job site. This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.
