29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On 31 July 2018, at approximately 8:45 AM, an employee was walking to their desk within the office and tripped or stumbled, falling to the floor, and requiring hospitalization from back and shoulder injuries. The employee was admitted on the same day, 31 July 2018, and the OSHA Austin Area Office was notified on 3 August 2018 at approximately 1:25 PM, or four days later.
