29 CFR 1910.134(k)(3): The employer did not provide the training prior to requiring the employee to use a respirator in the workplace: (Construction Reference: 1926.103)  a) 1200 Randolph Road, Plainfield, NJ 07060/Muhlenberg Hospital/Building 1  The employer did not ensure that all employees using half face respirators while performing work duties were provided adequate respiratory protection training to include elements covered in a written respiratory protection program such as but not limited to proper use, medical limitations and maintenance of such respirators before employees began to use them in the workplace.  Violation occurred on or about 8/01/2018.  WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

Employees used half face respirators provided by the employer, while removing copper pipes from ceilings in the building.    Violation occurred on or about 11/16/2018.    WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.1101(k)(10)(ii): The employer did not provide to the Assistant Secretary and the Director, upon request, all information and training materials relating to the employee information and training program:  a) 1200 Randolph Road, Plainfield, NJ 07060/Muhlenberg Hospital/Building 1  Information and materials on employee asbestos awareness training were not made available to the Assistant Secretary of Labor, upon request. Employees removed copper pipes from the basement at the site where an asbestos survey showed the presence of 5% Amosite asbestos and 15% Chrysotile asbestos in the basement pipe insulation of the building.  Violation occurred on or about 11/16/2018.  WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
